  • On-site encounter within 24 hours of receiving Notice-Of-Admission
  • Average of 3 encounters during average 5 day admission period.
  • Discharge encounter on day of discharge.
  • Written Discharge Summary/Personal Health Record provided to all designated providers of record within 48 hours of discharge.
  • Coordination of ancillary, social and medication services.
  • Coordination of timely physician office follow-up appointments,
  • Follow-up Post-Discharge Care telephonic encounter once a week for 3 weeks.
  • Follow-up In Home or Facility Encounter as per protocol


                                           SKILLED NURSING FACILITY

  • On-site encounter within 48 hours of receiving Notice-Of-Admission
  • Scheduled minimum of 4 in-facility  encounters during average 21 day admission period.
